Here’s how it works
-- Each combatant-to-be shares a photo or drawn reference for your character and for a knife of your choice
-- You can start with equal hp, or roll to see who gets a handicap or, if either one really wants to lose, agree on unequal hit points aka “health” and/or unequal dice sides. 40/40 works well for a couple hours' fight
-- Start writing a scene, how you came to be here and what's going on. Typically it's a conversation that turns to a confrontation but anything goes.
--For realism, attacks are consistent with the weapon chosen and damage goes with the hp lost.
-- To start the fight, someone describes an attack and rolls 1d20, one die with 20 sides for knives and for swords 1d25
-- An attack can be any physical move; it doesn't have to be with a knife; ideally the description is of an attempted attack, the move and where it’s aimed, since it might or  might not succeed.
-- Then the defender rolls and, if the attacker's roll is higher, the attack goes through and the defender takes damage equal to the difference between the rolls and describes the damage and their reaction.
-- Then the attacker describes a new attack and rolls again
-- If the defender rolls higher, then they describe their block or evasion and roll to attack and the attacker-become-defender rolls to block and launch their own attack or take damage
-- It goes on like that.
-- Tie rolls roll again unless you make a different rule
-- Error or cheat rolls, like rolling 1d30 lose regardless of the number
-- When one combatant goes under 20 hp, subsequent damage they take is +2
-- Below 10 hp a combatant becomes helpless and the victor can do what they want with them
-- Zero hp or lower is not death, it's helpless submission to the victor's will

Here's an Example
--◘ Attacker: "Die Mo-fo!" Rolls 1d20 to attack.
--◘ Now the defender rolls 1d20 and if the defender rolls higher, the defender describes their block and counterattack and rolls to attack.
--◘ If the defender rolls lower, then the defender's hp is decreased by the difference.
--◘ The defender writes their response based on the damage and the attacker describes a new attack and rolls to attack again.
--◘ After that first attack, the fight proceeds the same way.
--◘ Ties roll again unless it's agreed differently.
--◘ When a combatant's hp drops under 20, any damage is +2.
--◘ When a combatant's hp drops under 10, the combatant becomes helpless and the narrative resumes without dice. The loser belongs to the winner who does whatever they want with them.
--◘ There is no death at this point, even if the score goes negative, only helplessness. Death is up to the winner.
