Still looking for this but here are a few more details:

It would ten years on from the film, so around 1991. Isolated from the world, Arthur has no idea a new movement has been sparked by anarchists who follow by the concept of chaos and use him as a mascot. In Arkham state hospital, however, a new woman has been admitted. At 24, she is a frequent flier in the temp wards but for the first time she has been committed for her Borderline Personality Disorder, a potential misdiagnosis for what could be a more severe case. She is looking at longer term stay now and connects with Arthur, even though she only vaguely knows him as "that guy who killed that talk show host".

When the unknown group of followers attempt a breakout from the outside, Arthur leaves with the girl. Sparking a dysfunctional and obsessive relationship where she is the aggressor and pushes his darker side to the surface. Chaos and insanity reigns as they start a crime spree across the city of Gotham. Batman is optional...Bruce would still be in his teens so we would either have to play with his age or go with a different protagonist. Commissioner Gordon during his detective days, perhaps?

This is going to be a somewhat more real world game than comic book. The focus will be a lot on the female character as the one who pushes Arthur further down and the Joker back up. Expect screaming fights, throwing objects and headfuckery. Especially sparking jealousy and playing on insecurities about her leaving, leading him to strive to reach greater lengths to keep her interested. Which means more chaos!
