Do system games where combat can turn ugly easily go on Extreme by default?

Exactly what it says.

A GURPS game with the right set of rules can turn gritty real fast. There's already a rule that says that if you take enough damage your body is destroyed, and modern fighting can get there easily... A big enough IED can leave a real mess pretty reliably. So, if I run a game that is very likely to have messy deaths, even if they're not sexualised, is it Extreme by default?

Gore would go in Extreme, yes.  So if there is going to be gore described it should go in Extreme.

The board description for the Extreme games is as follows: "Games encompassed in a single thread involving elements that may squick out even non-con fans (such as bestiality, bathroom play, and gore)"

So, I think it would depend on what is detailed in your game. The way I see it is if it would be rated R, then stick it in Extreme. If they're going to be graphic deaths like in Game of Thrones, then I would do Extreme just to be safe.
